All-on-4 vs. All-on-6 Dental Implants: Which Full-Arch Solution Is Right for You?

All-on-4 vs. All-on-6 Dental Implants: Which Full-Arch Solution Is Right for You?

Your Dental Implant Options at Vero Dental Spa

You have done the research. You know implants are the permanent answer to missing teeth. What you may not yet know is which type of full-arch implant system, All-on-4 or All-on-6, is right for you.

That is a question worth taking seriously. Both options can restore an entire arch of teeth in a single procedure. Both offer stability that traditional dentures simply cannot provide. But the differences between them, in implant positioning, bone requirements, long-term durability, and candidacy, matter when you are making a decision that will affect how you eat, speak, and smile for the rest of your life.

Dr. Jonothan Royal is a Diplomate of the International Dental Implant Association with over 27 years of experience placing full-arch restorations in South Florida. This is his straightforward breakdown of both options, so you can walk into a consultation already knowing what questions to ask.

What Is All-on-4?

All-on-4 uses four dental implants, two placed vertically at the front of the jaw, and two placed at an angle toward the back, to support a fixed, full-arch prosthesis. The angled placement of the posterior implants is intentional: it allows the implant to anchor into denser areas of the jawbone that often remain intact even after years of tooth loss.

That design is also what makes All-on-4 eligible for same-day loading in many cases, meaning the prosthesis can be attached to the implants the same day they are placed. For patients who have spent years managing missing teeth or ill-fitting dentures, this is often a life-changing possibility.

What Is All-on-6?

All-on-6 uses six implants distributed across the arch, two additional implants compared to All-on-4, placed in the mid-arch. The increased implant count distributes bite force more evenly across the prosthesis, providing a higher level of structural stability and load-bearing capacity.
For patients with adequate bone density, All-on-6 is often the more durable long-term solution. The additional anchor points reduce stress on each implant, which can extend the functional life of the restoration and lower the risk of implant-level complications over time.

All-on-6 typically requires more available bone volume than All-on-4. For patients whose jawbone has experienced less resorption or who have completed bone grafting to restore lost volume, All-on-6 may offer a stronger structural foundation.

How Dr. Royal Determines Which Treatment Is Right for You

The choice between All-on-4 and All-on-6 is not made from a brochure; it is made from a 3D CBCT scan of your jaw, a review of your bone density and distribution, an assessment of your bite forces, and a conversation about your goals and timeline.

Dr. Royal has placed full-arch restorations for over 27 years. The variables he evaluates go beyond bone volume: the quality of your remaining bone, the position and anatomy of your sinuses and nerve canals, your health history, and the type of prosthetic material you want all factor into the recommendation. In many cases, the answer is clear. In others, he may recommend bone grafting first to make you a stronger candidate for the more durable All-on-6 approach.

No two full-arch cases are identical. At Vero Dental Spa, no two treatment plans are the same.

Vero Dental Spa’s Teeth Next Day® Treatment

For patients who qualify, whether they choose All-on-4 or All-on-6, Vero Dental Spa offers Teeth Next Day®: a proprietary full-arch restoration program designed to minimize the gap between implant placement and a fully functioning, natural-looking smile.

Traditional full-arch implant timelines can span months, with patients wearing temporary prosthetics while osseointegration occurs. With Teeth Next Day®, Dr. Royal’s team fabricates and places a precision prosthetic the day after your implants are seated. You leave with permanent-looking teeth. Not a placeholder.

Teeth Next Day® is available with both All-on-4 and All-on-6 protocols for eligible candidates. Candidacy is determined at consultation, based on bone density, implant stability at placement, and health factors that affect healing.

Frequently Asked Questions

Is All-on-6 always better than All-on-4?

Not necessarily. All-on-4 was specifically engineered to deliver excellent outcomes for patients with bone loss who might otherwise need extensive grafting. The right choice depends on your anatomy, not a ranking of options.

Can I get All-on-4 or All-on-6 if I have had bone loss?

Bone loss is actually one of the primary reasons patients are good candidates for All-on-4, which uses angled implants to find denser bone. Patients with significant bone loss may require grafting before an All-on-6 procedure. Dr. Royal evaluates this at the consultation using a 3D scan.

All-on-4 and All-on-6 are both proven, life-restoring solutions for full-arch tooth loss. At Vero Dental Spa, Dr. Royal doesn’t recommend a protocol because it’s popular. He recommends what the 3D imaging, bone assessment, and honest clinical judgment support.
