Scaling and Root Planing (Deep Cleaning)

Scaling and root planing is the gold standard, first-line treatment for periodontal disease. During the scaling phase, we use specialized ultrasonic and hand instruments to remove plaque, bacteria, and hardened calculus from the tooth surfaces and from beneath the gumline, reaching down to the bottom of infected pockets that normal brushing and even regular cleaning cannot touch. Root planing follows, in which we carefully smooth the root surfaces of your teeth, removing bacterial toxins embedded in the cementum and creating a clean, biocompatible surface that discourages bacterial recolonization and allows the gum tissue to reattach.

The procedure is typically divided into quadrants and performed with local anesthesia to ensure you’re comfortable throughout. Most patients require two appointments (one side of the mouth per visit) to complete treatment. We schedule a follow-up evaluation 4 to 6 weeks later to measure your response to treatment.

Biolase® Waterlase Laser Enhancement

At Vero Dental Spa, SRP is enhanced with the Biolase® Waterlase. This dental laser system uses a patented combination of laser energy and water to precisely remove diseased tissue and bacterial biofilm from periodontal pockets without damaging the surrounding healthy tissue. The Waterlase also promotes the release of growth factors that support faster healing of the gum tissue post-treatment.

Our use of the Biolase® Waterlase represents an advantage that results in less post-treatment inflammation, reduced bleeding, and, for many patients, a more comfortable recovery.

Antibiotic Therapy (Targeted Site Treatment)

For cases with active infection in specific pockets, Dr. Royal may place a locally delivered antibiotic directly into the periodontal pocket following scaling and root planing. These time-release microspheres release antibiotic medication over 7 to 10 days, targeting bacteria that may persist in the pocket after the mechanical cleaning.

Unlike systemic antibiotics (taken by mouth), site-specific delivery concentrates medication where it’s needed while minimizing systemic side effects. No removal is required; the material is completely bioresorbed.

Periodontal Maintenance Program

After active therapy, gum disease doesn’t simply disappear; it requires ongoing management. Bacteria re-colonize cleaned pockets within 90 days, which is why we transition patients to a personalized periodontal maintenance schedule (typically every 3 to 4 months). These appointments are more thorough than routine cleanings: we re-chart pocket depths, monitor bone levels, perform targeted cleaning in previously affected areas, and assess your home care routine.

Consistent maintenance is the single most important factor in long-term periodontal health.

Does insurance cover non-surgical periodontal treatment?

Most PPO dental plans cover scaling and root planing when a clinical diagnosis of periodontal disease is documented, typically at 50–80% after your deductible. The Biolase® Waterlase is a dental laser system that uses a patented YSGG laser combined with water and air spray to precisely remove diseased tissue and decontaminate bacterial biofilm from periodontal pockets. It causes less thermal damage than traditional lasers, promotes tissue healing, and reduces post-treatment inflammation. Most patients find their recovery more comfortable when a laser is added to their treatment.

Will I need to return for follow-up appointments?

Yes, and this is critically important. We schedule a re-evaluation 4 to 6 weeks after your final SRP appointment to measure pocket depths and assess healing. After that, you’ll transition to a personalized periodontal maintenance program (typically every 3 months) to prevent relapse. Think of it like a maintenance plan for your gums, not a one-and-done treatment.

What can I do at home to help my gum disease treatment succeed?

Home care makes or breaks your treatment outcomes. We’ll review your brushing and flossing technique in detail and may recommend adjuncts such as a soft-bristle or electric toothbrush, interdental brushes for large spaces, and a prescription-strength fluoride or antimicrobial rinse. Quitting smoking has the single largest positive impact on periodontal healing. Managing blood sugar (for diabetics) is also critical to reducing inflammation.
